Dementia Care and Education Campus | Dementia Care and Education Campus

Programs that provide a full range of supportive services in an inpatient or residential setting for terminally ill people who are in the final stages of their illnesses and for their families. Services may include medical care, pain and symptom management, home nurse visitation, case management, emotional and spiritual support, and bereavement services for the patient and members of the family.

Hospice Facilities

Programs that provide for the comprehensive management of the physical, psychological, social, spiritual and existential needs of patients with the objective of helping them achieve the best possible quality of life through relief of suffering, control of symptoms and restoration of functional capacity while remaining sensitive to personal, cultural and religious values, beliefs and practices. The focus of palliative care is on people with incurable, progressive illnesses who are in the early stages of their illness. Palliative care may be provided in a wide variety of health care settings including hospitals, nursing facilities, hospice facilities and the person's own home; and may be coordinated with other services including curative therapies.

Palliative Care

Separate units of long term care facilities including hospitals that focus on care of patients with a particular diagnosis such as Alzheimer's disease, dementia, head injuries or other specific disorders.

Special Care Inpatient Units

Programs that offer any of a variety of therapeutic approaches which are intended to maximize the existing cognitive functioning of people who have Alzheimer's disease, Pick's disease or other forms of cognitive impairment. Interventions include structured activities and exercises that are designed for people who have a short attention span and require extra verbal and visual stimulation with the objective of enhancing the individual's perception of the environment, promoting trust, reducing anxiety, avoiding overstimulation and maximizing communication skills.

Dementia Management

Supports families living with all types and all stages of dementia. Provides a 10-bed dementia Hospice Inpatient Home that serves people with advanced disease who are receiving hospice care. Also provides a 12-bed Assisted Living Home for people with mild and moderate dementia, who may or may not be on hospice. Offers assistance to manage symptoms when 24/7-short-term acute care is needed.

Levine Inpatient Care Home | Levine Inpatient Care Home

Provides memory cafes, support groups, and community education and engagement activities for people with dementia. Also provides free dementia screenings.

Dementia Capable Southern Arizona | Dementia Capable Southern Arizona

Programs that offer simple tests for the early detection of mild memory problems that may not be associated with Alzheimer's disease or related dementias. The screenings are generally confidential and involve completion of a questionnaire and a short memory test. People who exhibit cognitive and behavioral problems beyond those associated with normal aging may be referred for neuropsychological testing.

Memory Screening

Mutual support groups whose members are individuals in the later years of their lives. The groups meet in-person, by telephone or via the Internet; and allow participants to share their experience, strengths and hopes to solve the problems they have in common and to address the quality of life issues that affect them all.
