Provides case management for individuals and families with low income. Services may include one time utility assistance and/or utility deposits, gas vouchers, food vouchers, local bus/taxi fare and limited rent and mortgage payment assistance. Rental assistance only available for seniors at this time.

Programs that develop plans for the evaluation, treatment and/or care of individuals who, because of age, illness, disability or other difficulties, need assistance in planning and arranging for services; which assess the individual's needs; coordinate the delivery of needed services; ensure that services are obtained in accordance with the case plan; and follow up and monitor progress to ensure that services are having a beneficial impact on the individual. Case management is a collaborative process characterized by communication, advocacy and resource management to promote high quality, cost-effective interventions and outcomes.

Case/Care Management

Programs that supply food coupons which can be exchanged in designated grocery stores, supermarkets and/or farmers markets for food products. The vouchers are generally provided to low income individuals and families on an occasional or ongoing basis, but may also be available to other specified populations; and may be issued in paper or electronic formats.

Food Vouchers

Programs that provide cash grants or loans for people who need to have their electricity, gas, water or telephone service turned on but do not have the assets to pay the required deposits. Utility deposits are not required everywhere.

Utility Deposit Assistance

Programs that pay all or a portion of the home heating fuel expenses of people who are unable to purchase the resources to heat their homes without assistance. Also included are non-emergency programs like those funded through the federal Low Income Home Energy Assistance Program (LIHEAP), also referred to as LIEAP or HEAP in some states, that provide home energy assistance for low-income households that apply, usually once per calendar year; and programs that pay for firewood, propane, butane, kerosene, coal or other bulk fuels that are used for heating or cooking purposes in situations where people have no other means of acquiring them. Heating fuel payment assistance programs may have age, income, disability, need or other eligibility requirements.

Programs that pay all or a portion of the gas service expenses of people whose gas has been or is at risk of being shut off including any reconnection fees that may apply. Also included are non-emergency programs like those funded through the federal Low Income Home Energy Assistance Program (LIHEAP), also referred to as LIEAP or HEAP in some states, that provide home energy assistance, generally in the form of a credit, for low-income households that apply. The assistance is usually available once per calendar year (or heating season). Gas service payment assistance programs may have age, income, disability, need or other eligibility requirements.

Programs that pay all or a portion of the electric service expenses of people whose electricity has been or is at risk of being shut off including any reconnection fees that may apply. Also included are non-emergency programs like those funded through the federal Low Income Home Energy Assistance Program (LIHEAP), also referred to as LIEAP or HEAP in some states, that provide home energy assistance, generally in the form of a credit, for low-income households that apply. The assistance is usually available once per calendar year (or heating season). Electric service payment assistance programs may have age, income, disability, need or other eligibility requirements.

Electric Service Payment Assistance

Programs that pay all or a portion of the water service expenses of people whose water has been or is as risk of being shut off including any reconnection fees that may apply. Included are programs that provide assistance with combined water/sewer bills in communities where sewer payments are bundled with payments for water services. Water service payment assistance programs may have age, income, disability, need or other eligibility requirements.

Programs that provide cash, vouchers or discount coupons that individuals can use to pay for taxi services in situations where they have no personal means of transportation, are unable to use public transportation for necessary local travel but do not need the capacities of a paratransit service.

Taxi Fare

Programs that pay for or provide maintenance and repair services to people who have a large or major appliance that is not working properly. A large or major appliance is a non-portable or semi-portable machine used for routine housekeeping tasks such as cooking, washing laundry or dishes, or food preservation. Large appliances are often considered fixtures and part of real estate and as such are often supplied to tenants, along with their maintenance and repair, as part of otherwise unfurnished rental properties. Major appliances may have special electrical connections, connections to gas supplies, or special and permanent plumbing and ventilation arrangements which limits where they can be placed in a home.

Large Appliance Repair

Programs that provide financial assistance and supportive services that stabilize households in their current housing or help them to move into new housing without first entering the shelter system or experiencing homelessness. Services may include financial assistance (short or medium-term rental assistance, security or utility deposits, utility payments, moving cost assistance), housing relocation and stabilization services, legal assistance (generally mediation), credit counseling and case management. The goal is to help households resolve their crisis, secure short-term financial or rental assistance as needed, and access ongoing sources of support in the community in order to remain housed. If the individual or family is unable to stay in their existing housing, the prevention program helps the household to find an alternative housing arrangement that is safe, reasonably affordable and adequate.

Homelessness Prevention Programs

Private, nonprofit human service and advocacy organizations established under the Economic Opportunity Act of 1964 that provide emergency assistance to people in crisis; address the causes of poverty through programs that fight unemployment, inadequate housing, poor nutrition and lack of educational opportunity; and provide training in advocacy skills. Community Action Agencies (CAAs), also known as Community Action Programs (CAPs), operate a variety of core services in the communities they serve. Included are GED and ESL education, job training programs, Head Start and day care, weatherization and energy assistance programs, health services, services for older adults (e.g., Foster Grandparents and RSVP), emergency food assistance and homelessness prevention programs. They are also involved in affordable housing creation and renovation and small business assistance.

Community Action Agencies

Programs that provide money or vouchers to purchase gasoline, most frequently for individuals who need their automobile for necessary local or out-of-town travel but cannot afford to run their vehicle without assistance, but occasionally for people driving through the local area who have become stranded.

Gas Money

Programs that provide cash or tokens for individuals who have no personal means of transportation and are unable to use public buses for necessary local travel without assistance with the fare.

Local Bus Fare

Provides taxi vouchers to seniors and people with disabilities.
